IUPLSSS lunchtime conference: Implementation of trajectory management in Quebec: where are we?

Following a pilot project (2017-2019), a study funded by the Canadian Institutes of Health Research (2020-2025) aims to identify the different factors for implementing management by care and service trajectory (GTSS) in different CISSS and CIUSSS in Quebec, and to analyze them.
